Building Independence
Cardinal Services is proud to offer two residential options —community living and residential living — with varying levels of support to help individuals live the life they want.
Our community living services help adults with disabilities live as independently as possible by providing the support they need to build skills and achieve their goals.
Whether individuals live in their own homes or with family members, services are provided based on their specific needs. Direct support professionals assist with a wide range of life skills, such as taking medication, grooming, cooking, cleaning or handling finances.
Services may be as complete as full-time, 24-hour support, or as minimal as meeting with staff once a week – it all depends on the individual needs of the person.
Community living services are available in Cass, Fulton, Kosciusko, and Marshall counties.
Individuals must have the Community Integration and Habilitation Waiver or Family Support Waiver to participate in community living services. Those who do not have either Medicaid waiver may want to explore our residential living (group home) program.
Wellness Coordination
Individuals participating in community living services may also be eligible for wellness coordination. Wellness coordination is designed for individuals who need the assistance of a nurse to coordinate their medical care. It includes development and monitoring of an ongoing wellness plan.
Wellness coordination participants receive care based upon three tiers of need:
Tier 1
Face to face visit once a month with a nurse and consultations based on the individual need.
Tier 2
Face to face visit twice a month with a nurse and consultations based on the individual need.
Tier 3
Face to face visit once a week with a nurse and consultations based on the individual need.
